Registration of Live Birth:

On-time registration – (Payment P100.00)

(Requirements: Certificate of Live Birth, Marriage Certificate of parents)

Delayed registration of Live birth

(Requirements. PSA negative result, Joint Affidavit for delayed registration, baptismal, valid i.d, barangay certification for late registration, cedula, marriage certificate)

Out-of town reporting of birth

(Requirements: Affidavit for Out-of Town registration, barangay certification, baptismal, valid i.d, marriage certificate, cedula)

Legitimation of Illegitimate children – (Payment P200.00)

(Requirements: Advisory on Marriage/CENOMAR, Affidavit of Legitimation, marriage certificate of parents, cedula/ valid i.d. Affidavit of Acknowledgement)

Registration of Death:

Registration of Application for Marriage License:

(Requirements: CENOMAR, Birth Certificate or Baptismal Certificate, Sedula, Parental Consent or Advice if the applicant is below 25 years old, Legal Capacity to Contract Marriage if one of the contracting parties is foreigner, Certificate of Counselling, Barangay Certificate for Tree Planting)

Registration of Marriage:

Correction of Clearical or Typhographical Erroe in the Certificate of Live Birth and Certificate of Marriage :

RA 9048 – Processing fee – P1,000.00

Supporting Documents:

Certified machine copy of both LCRO and PSA copy of certificate containing the alleged erroneous entry or entries

Not less than 2 public or private documents upon which the correction shall be based

(Processing Fee- P3,000.00) + publication

Requirements:

*All required documents for Correction of Clerical Error shall be submitted also by the petitioner for Change of First Name

*Certified machine copy of birth both PSA and LCRO

*Employer certificate

*Affidavit of Publication from the publisher
